Introduction

 

Transition in the education community refers to the student’s movement through school into adult life. This can be exciting and rewarding for the student with a disability and his/her family. At the same time, it is a complex process that can be stressful, tiring, and frustrating. Students and their families should be aware that planning for a successful future and carrying out a transition plan is a combined effort of the student, parents, schools, and other agencies. The student’s interests, preferences, and special needs, including the levels of assistance and services needed, help to formulate a vision of the student’s future. Linkages with other agencies and support services can be explored early to gain information and to begin making choices.

 

Access to adult services may be very different from experiences with the school system. Referral, eligibility criteria, and funding mechanisms for services are established by each agency and may differ in important aspects. Finding out about adult services early allows time to gather information and make choices. Growing up is exciting and there are many opportunities for exceptional students to continue learning and to live, work, and play in the community.

 

Living arrangements and working considerations need to be considered hand in hand. Where the student works effects where he/she can live, access transportation, play, and participate in the community. Housing options and services vary. They may include:

 

·        Living with a family member or relative with no other support

·        Family support through help in the family’s home

·        Adult foster care

·        A personal care home or assisted living facility

·        A residential treatment center

·        A group home (public or private); services may range from part-time to 24 hours/7 days per week.

·        Semi-independent living, with or without a roommate

·        Personal care assistance

·        Independent living, with or without a roommate

·        Subsidized housing or low-interest home purchase, with or without support services

·        Various combinations of the above

 

The chapter on “Housing Options/Residential Services” may assist in the search for providers of the services needed by the student. Providers of services can often help in making decisions regarding options, as well.
